To ensure no adoptee loses access to important information about themselves and their heritage, Choice has taken the responsibility of storing both current and historical files from several organizations who have closed their doors.
On this page, you will find which agencies’ records we maintain and how to request your own files if you fit the criteria listed.

How to Retrieve Your Adoption Files
Adoptees over the age of 18 can contact Choice for a post-adoption search and to retrieve their physical adoption records. The cost is $150 per file retrieval, and the process can take up to four weeks. We cannot guarantee that files will contain the specific information requested, and we will not know what information is available until we retrieve and review the file.If you are a birth parent who gave birth in Oregon and your child is 18 or older, you can search for them through the Oregon Voluntary Registry.
We cannot release information in cases involving a closed adoption.
Cases for Adoption Mediation
​Choice provides adoption mediation for adoptive families and birth parents whose child is under 18, the adoption is open, and the post-adoption contact agreement includes mediation. This includes families who worked with Adventist Family Services, Christian Family Adoptions, PLAN, Tree of Life Adoption Services, or Choice Adoptions.

Mediation may include facilitating communication between parties, passing information between birth and adoptive families, maintaining files, providing access to eligible records, and maintaining or updating post-adoption contact agreements when necessary. Services are provided in accordance with applicable laws, confidentiality requirements, and contact agreements between parties. Adoption mediation through Choice includes a one-time $500 fee, which allows our social workers to serve as a liaison and provide ongoing support to both sides as needed.
